Congratulations all families who have matched with BASIS DC in the MySchoolDC Lottery. We look forward to having you join our BASIS DC community.

See below for the next steps that you need to complete to secure your student’s seat for the 2024-25 school year.

All registration steps must be completed and submitted by May 1st.



Step 1: SchoolMint Registration

All newly matched students begin their registration process through SchoolMint. SchoolMint registration must be filled out by May 1st.

Please follow the below instructions carefully to complete SchoolMint Registration.

  • Create a SchoolMint account: https://applybasis.schoolmint.com/signup
  • Click Get Started next to “Add a student” on the Welcome Page
  • Under “Student Lookup,” enter your student’s MySchool DC ID that was emailed to you from Ms. Malika Walters
  • Next, you will be redirected to the general registration form.
  • Fill out the entire online form—you cannot print and mail your response.
  • Review and submit – make sure you get to the page that says, “review and send.”
  • You will receive an email confirmation after submitting the online form.

Please make sure that you receive a confirmation email after completing the SchoolMint registration. If you do not receive an email, you are not done.



Step 2: Submit Required Residency Verification and Initial Intake Forms via Xodo sign

You will receive two additional emails from Malika Walters via [email protected] with access to your 2024-2025 District of Columbia Residency Verification form and requesting a one-time upload of your State ID and your student’s Birth Certificate.

Your signature and upload of all necessary documents is required to complete your BASIS DC 2024-25 school registration.  You will receive both upload links soon if you have not already!



Step 3: Medical and Dental Forms via Smartsheet Form

All newly matched students must submit medical and dental forms.

Medical and Dental Forms must be submitted for Derrick by May 1st. Please follow the below instructions carefully to complete the medical and dental forms upload.

  • Click on the following form link: https://tinyurl.com/3fsdn8vf
  • Fill out the entire online form—you cannot print and mail your response.
  • Review and submit – make sure that all necessary forms are attached!
  • You will receive an email confirmation after submitting the online form.

The date of the exam MUST be within the last twelve months for medical and dental forms. All families must submit at least medical and dental forms. Parents of students with food allergies or asthma are encouraged to submit an Action Plan for Anaphylaxis and/or Asthma.

Step 4: Summer programs

Once you are registered, we do offer summer programming for all students, including 5 session options for the summer BOSS camp “BASIS Organizational Skills for Success.” You will receive the flyer with signup instructions next week.
